  • Patient Forms

    We know you're busy, so we do our best to run our offices on time. Since we gather most of your contact and insurance information when you make your appointment, there's very little additional paperwork to be completed when you arrive for your first visit. However, please be sure to bring your insurance card, driver's license or photo ID, and a list of your current medications to your appointment.

    During your first visit, a nurse will check your vital signs and review your medical history. Then you'll meet with your doctor, who will assess your symptoms and medical condition. The doctor will listen to your heart, pulses, lungs and carotids, and may also review your family medical history with you. If more diagnostic information is needed, additional tests will be ordered. When possible, those tests will be performed during your appointment, and you'll receive the results immediately. Your doctor may prescribe medication(s) for you and/or make other suggestions to improve your health. You may be asked to make a follow-up teaching appointment with your doctor to become more knowledgeable about your condition.

    If additional procedures are required, our front desk staff will schedule them while you're still in the office. We'll do everything possible to make your appointments as convenient as possible.

    As a courtesy, we routinely send follow-up reports to referring physicians. Whether we're making additional appointments for you, referring you to another doctor, or following up on your test results, Texas Health Heart & Vascular Specialists always goes the extra mile to make sure you receive the expert care and support you deserve throughout your treatment.

  • Insurance Information

    Texas Health Heart & Vascular Specialists accepts a wide range of insurance plans and various forms of payment, and will verify benefits prior to your appointment. We believe in making it easy for our patients to focus on their treatment and regaining good health. Please call us at 940-320-2188 if you have any questions.

  • Pay a Bill
